The conversion is actually:
  - add blank lines and identation in order to identify paragraphs;
  - fix tables markups;
  - add some lists markups;
  - mark literal blocks;
  - adjust title markups.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

Also, removed the Maintained by, as requested by Geert.

DocBook is mentioned several times at the documentation. Update
the obsolete references from it at the DocBook.

This API will be used to support YUV frame buffer formats in a standard
way.

Last but not least, create a much needed fbdev API documentation and
document the format setting APIs.
